The organization provides personalized in-home care services designed to help elderly individuals maintain their mobility, functional independence, and quality of life while receiving care in the comfort of their own homes.

Role Description

This is a full-time, home-based role for a Physiotherapist – Geriatric Care based in Kathmandu Valley. The Physiotherapist will conduct in-home assessments and provide personalized physiotherapy and rehabilitation services to elderly patients with age-related mobility limitations, musculoskeletal conditions, neurological conditions, post-surgical needs, balance and gait difficulties, and general physical deconditioning.

Responsibilities include assessing patients’ physical and functional abilities, developing individualized treatment plans, providing therapeutic exercises and rehabilitation interventions, conducting mobility, balance, and gait training, and educating patients and caregivers on safe mobility, home exercises, fall prevention, and ongoing rehabilitation.

The Physiotherapist will monitor patient progress, maintain accurate and confidential clinical records, communicate with family members and caregivers, and coordinate with doctors, nurses, care coordinators, and other healthcare professionals when required to ensure safe and comprehensive patient care.

The role requires the ability to work independently in patients’ homes while maintaining a professional, compassionate, and patient-centered approach.

Key Responsibilities

* Conduct comprehensive physiotherapy assessments for elderly patients in their homes.

* Assess pain, range of motion, muscle strength, mobility, balance, gait, functional independence, and fall risk.

* Develop individualized physiotherapy and rehabilitation plans based on each patient’s condition and functional goals.

* Provide therapeutic exercises, strengthening, stretching, mobility, balance, gait, and functional training.

* Provide appropriate rehabilitation for orthopedic, neurological, post-surgical, and age-related conditions.

* Educate patients and family members/caregivers regarding home exercises, safe transfers, mobility, positioning, and fall prevention.

* Recommend and provide training on the appropriate use of mobility and assistive devices such as walkers and canes when clinically appropriate.

* Monitor and document patient progress and modify treatment plans as needed.

* Maintain accurate, timely, and confidential patient assessment and treatment records.

* Communicate significant changes in a patient’s functional or physical condition to the appropriate family member or healthcare professional.

* Collaborate with Saathi Sneha Care’s care coordinators, caregivers, nurses, doctors, and other healthcare professionals to support continuity of care.

* Maintain professional standards, patient confidentiality, infection-control practices, and appropriate boundaries during home visits.

* Provide compassionate, respectful, and culturally sensitive care to elderly patients and their families.

Qualifications

* Bachelor’s degree or higher in Physiotherapy.

* Valid Nepal Health Professional Council (NHPC) registration/license to practice as a Physiotherapist in Nepal.

* Strong knowledge of therapeutic exercises, geriatric rehabilitation, orthopedic rehabilitation, and neurological rehabilitation.

* Strong patient assessment, clinical reasoning, treatment planning, and rehabilitation skills.

* Ability to assess mobility, gait, balance, functional limitations, and fall risk in elderly patients.

* Ability to educate patients, family members, and caregivers on rehabilitation programs and preventative care.

* Strong interpersonal, communication, and patient-care skills.

* Ability to work independently during home visits while maintaining appropriate professional standards.

* Good documentation and record-keeping skills.

* Ability to travel to patients’ homes within Kathmandu Valley.

* Prior experience working with elderly or geriatric patients, home-based physiotherapy, neurological rehabilitation, or orthopedic rehabilitation is preferred.

* CPR/First Aid certification is an advantage.
